Cigna Beats Q3 Estimates and Raises Its 2026 Outlook
Direct answer
The Cigna Group, a large US health company, reported better than expected results for the third quarter of 2026 on November 5, 2026. Adjusted profit was $7.98 per share, above the $7.52 analysts expected, and the company raised its profit forecast for the full year.
What happened
Cigna reported adjusted revenue of $73.8 billion for the quarter ended September 30, 2026, up 6.1 percent from a year earlier and above the $72.64 billion analysts expected, according to Zacks. Total revenue was $73.7 billion, up 6 percent, the company said in its own results release.
Adjusted earnings per share were $7.98, up 1.9 percent from $7.83 a year earlier and about 7 percent above the consensus estimate. Reported net income was lower, at $1.4 billion or $5.47 per share, down from $1.9 billion or $6.98 per share a year earlier, mainly because last year’s quarter included a gain on a sale that did not repeat.
The health insurance business did most of the work. Cigna Healthcare revenue rose 8.9 percent to $11.7 billion, and its pre-tax adjusted operating income rose 14.9 percent to $1.2 billion. Medical customers stood at 18.5 million, up 2 percent from a year earlier.
Evernorth Health Services, which includes the pharmacy benefit business, grew adjusted revenue 5.6 percent to $63.8 billion, while its pre-tax adjusted operating income fell 3.9 percent to $1.8 billion. The medical care ratio, the share of premiums spent on medical care, was 85.0 percent, close to 84.8 percent a year earlier.
Cigna raised its 2026 outlook. It now expects adjusted earnings of at least $30.60 per share for the full year, up from at least $30.45 before. Shares slipped about 1 percent on the day of the report.
Why it matters for beginners
A company beats estimates when its profit comes in above what analysts expected, and that is often a sign the business is running well. Here the insurance arm grew strongly while the pharmacy business was softer, so the result is not strong in every part of the company.
An outlook raise matters because it is the company’s own forecast. Cigna lifted it after nine months of results, which tells investors management expects the good run to continue into the end of the year.
A lower reported profit next to a higher adjusted profit can look confusing. Adjusted figures strip out one-off items, such as last year’s sale gain, so they show the underlying business more clearly. Both numbers are real, they just answer different questions.

What to watch next
Whether medical costs stay in line with pricing, since the medical care ratio was little changed and management will be judged on it next quarter.
Whether the pharmacy benefit business recovers, after its profit fell in the quarter.
Whether Cigna reaches the raised forecast of at least $30.60 per share for 2026, and what it says about 2027.
Frequently asked questions
What is Cigna?
The Cigna Group is a US health company with two main parts: Cigna Healthcare, which sells health insurance, and Evernorth Health Services, which runs pharmacy benefit and care services.
Why did Cigna beat estimates?
Adjusted profit of $7.98 per share beat the $7.52 consensus, helped by strong growth in the Cigna Healthcare insurance business, where revenue rose 8.9 percent and operating income rose 14.9 percent.
What is adjusted earnings per share?
Adjusted earnings per share is profit per share after removing one-off items, such as gains on sales or restructuring costs. It is meant to show how the regular business performed.
Why did reported profit fall while adjusted profit rose?
Reported net income fell to $1.4 billion from $1.9 billion a year earlier because last year’s quarter included an after-tax gain on a sale. Adjusted profit, which removes that kind of item, rose slightly.
Does a beat mean the shares are a good buy?
No. A beat reflects one quarter’s results. It does not promise future gains, and shares can fall even after a good quarter.
